Experience Safety Cross
The ward's month on a green cross; a red day summons a leader.
The job
Gives each unit a daily picture of the days patients reported as bad, ready for the morning huddle, and brings a leader to the ward within 24 hours when its patient experience signs get worse.
The moment
Each ward's month is shown as a cross-shaped calendar, one cell per day, coloured green or red by what patients report.
Ward 7B has 19 green days in a row. On day 20 the cell turns red, at a score of 6, because three patients said their pain was ignored overnight.
The 06:30 alert has already paged the Director of Nursing. By 11:00 the Director has visited, and a note from the ward round sits on that day's cell.
The next nine days are green, and on the first of the month the printed cross comes out of the staff-room printer.
