The job
Measures and closes the gap, unit by unit, between the safety events patients describe and the incidents staff actually reported, as a measure of reporting culture. Hands surveyors the evidence before they ask.
The moment
At the quality council, each hospital unit is drawn as an iceberg. Above the waterline are incidents staff reported. Below it are events patients described that were never reported.
Surgical ward 5 shows 9 reported falls above the water and 14 falls described by patients below it.
The unit head files five missing reviews during the meeting, and five blocks float up above the waterline.
Next quarter, the part under water is down to 3. At the accreditation survey, the surveyor asks to see "the iceberg" by name and is handed the unit's standards page.
